ПЕРИТОНЕАЛЬНИЙ ДІАЛІЗ У ЛІКУВАННІ ПАЦІЄНТІВ З ГОСТРИМ ПОШКОДЖЕННЯМ НИРОК
Анотація
Робота є оглядом літератури. Перитонеальний діаліз (ПД) був першим методом діалізної ниркової замісної терапії (ДНЗТ), застосованим для лікування пацієнтів з гострим пошкодженням нирок (ГПН). ПД здатний коригувати метаболічні, електролітні, кислотно-лужні розлади та гіперволемію у хворих на ГПН. Постійний високооб’ємний ПД та постійний проточний ПД можуть забезпечити діалізну дозу порівняну з екстракорпоральними методами ДНЗТ. Проте, коригуючий вплив методики на більшу частину складових синдрому ГПН за потужністю поступається гемодіалізу. У зв’язку з цим ПД може бути застосований у пацієнтів з нестабільною гемодинамікою, ризиком кровотеч та/або рефрактерною серцевою недостатністю, а також в умовах обмежених ресурсів, коли екстракорпоральні діалізні методики є недоступними. Крім того, ПД є життєздатною альтернативою гемодіалізу у дітей з ГПН. Саме тому, вибір лікаря на користь застосування ПД для лікування хворих на ГПН має враховувати особливості конкретної клінічної ситуації, досвід персоналу у застосуванні методів ДНЗТ та їх доступність.
